Applying for rehabilitation from the German pension insurance (G0100)
Form G0100 is how insured persons apply to the German statutory pension insurance (Deutsche Rentenversicherung) for participation benefits (Leistungen zur Teilhabe) — above all medical rehabilitation. The statute puts rehabilitation BEFORE a pension: participation benefits take priority over pension benefits (§ 9(1) sentence 3 SGB VI, the "Reha vor Rente" principle). Two hurdles decide: the personal one (earning capacity considerably at risk or reduced, § 10(1)) and the insurance one (§ 11). For the self-employed the trap lies in the second: the short route of six calendar months of COMPULSORY contributions in the last two years (§ 11(2) no. 1) is not open to someone who is only voluntarily insured.

Deadlines
- The date of application sets the look-back window
- § 11(1) SGB VI tests the conditions "at the time of application"; subsection 2 no. 1 counts compulsory contributions "in the last two years before the application" — an application left lying shifts that window
- Four years between two medical rehabilitations
- § 12(2) SGB VI — the only exception is where earlier benefits are urgently needed on health grounds
- Duration: three weeks at most, extendable
- § 15(2) sentences 3 and 4 SGB VI — longer where needed to reach the rehabilitation goal
- 14 days to choose the facility
- § 15(6a) sentence 5 SGB VI — where the pension body proposes facilities because the insured made no proposal or the proposal does not meet the criteria

Who this applies to
- Insured persons whose earning capacity is considerably at risk or reduced by illness or disability, and for whom the benefit is expected to avert a reduction or substantially improve earning capacity (§ 10(1) SGB VI)
- Compulsorily insured self-employed persons with at least six calendar months of compulsory contributions for an insured activity in the two years before applying (§ 11(2) no. 1 SGB VI) — for medical rehabilitation
- Anyone who, when applying, has completed the 15-year qualifying period or draws a pension for reduced earning capacity (§ 11(1) SGB VI); calendar months with contribution periods count towards the 15 years (§ 51(1)), and contribution periods include periods of voluntary contributions (§ 55(1) sentence 1)
- Voluntarily insured self-employed persons without those 15 years: for medical rehabilitation only if their earning capacity is reduced, or is expected to be in the foreseeable future, and they have completed the general qualifying period (§ 11(2) no. 3 SGB VI)
- NOT anyone who can obtain equivalent benefits from another rehabilitation body because of an accident at work or an occupational disease, who draws or has applied for an old-age pension of at least two thirds of the full pension, or who draws a benefit paid regularly until an old-age pension begins (§ 12(1) nos. 1, 2 and 4a SGB VI)
- NOT for a further medical rehabilitation within four years of a publicly funded or subsidised one — unless it is urgently needed on health grounds (§ 12(2) SGB VI)
Worked example
Sabine Kowalski, self-employed master joiner, and her voluntarily insured colleague
Sabine has run a joinery alone for years and is compulsorily insured in the pension scheme as a self-employed person. A disc problem threatens her work at the bench; her GP recommends rehabilitation. Personal condition: her earning capacity is considerably at risk, and medical rehabilitation is expected to avert a reduction (§ 10(1) SGB VI). On the insurance side, six calendar months of compulsory contributions in the two years before applying are enough (§ 11(2) no. 1) — she has twenty-four. She applies for medical rehabilitation on form G0100 and proposes a clinic (§ 15(6a) sentence 1); her doctor prepares the medical report S0051 (Befundbericht), which goes with the application. During the stay — as a rule three weeks at most (§ 15(2) sentence 3) — the workshop stands still. Sabine earned income from self-employment immediately beforehand and paid contributions in the assessment period: she is entitled to transitional allowance (Übergangsgeld) (§ 20(1) no. 1 and no. 3(a)). If her contributions for the last calendar year were based on income of EUR 42,000, the calculation base is 80 per cent of it, i.e. EUR 33,600 (§ 21(2)); the rest of the calculation follows Book IX of the Social Code (§ 21(1)). Her colleague Timo has paid only voluntary contributions for eight years. The two-year rule does not help him: voluntary contributions count there only if they are treated as compulsory ones (§ 11(2) sentence 2 with § 55(2) no. 1). He lacks the 15 years of § 11(1). Medical rehabilitation is open to him only if his earning capacity is reduced or is expected to be soon — his eight contribution years satisfy the general qualifying period (§ 11(2) no. 3, § 50(1), § 51(1)).
How to fill it in, step by step
- Establish with your doctor whether your earning capacity is considerably at risk or reduced and whether rehabilitation is expected to preserve or improve it (§ 10(1) SGB VI).
- Check your insurance record: six calendar months of compulsory contributions in the last two years, the 15-year qualifying period, or — if your earning capacity is reduced — the general qualifying period (§ 11 SGB VI).
- Check the exclusions, in particular a medical rehabilitation in the last four years (§ 12(2) SGB VI) and responsibility of the accident insurance (§ 12(1) no. 1).
- Ask your pension insurance body whether it needs a medical report or a medical expert opinion — procedures differ from one body to another.
- Have your treating doctor complete and sign the medical report S0051.
- Complete form G0100 and, if you wish, propose rehabilitation facilities (§ 15(6a) sentence 1 SGB VI).
- Attach the relevant annexes from the DRV form package: G0110 (annex to the application for medical rehabilitation), G0105 in cross-border cases, and questionnaire F0870 where claims for damages may exist.
- Sign the application.
- Submit it and await the social-medical assessment; once approved, choose among any facilities proposed to you within 14 days (§ 15(6a) SGB VI).
Good to know
- "Rehabilitation before pension" is in the statute: participation benefits take priority over pension benefits that would not be payable, or only later, if participation succeeds (§ 9(1) sentence 3 SGB VI).
- The rehabilitation application can become a pension application: it counts as one where the insured's earning capacity is reduced and success is not to be expected, or the benefits did not prevent the reduction (§ 116(2) SGB VI).
- For the self-employed the transitional allowance is the crucial point: it is due to anyone who earned income from work immediately before the incapacity or the benefit and paid contributions in the assessment period (§ 20(1) no. 3(a) SGB VI); its calculation base is 80 per cent of the income on which the contributions for the last calendar year were based (§ 21(2)).
- § 21(2) SGB VI looks at the income underlying the contributions ACTUALLY PAID: contributions paid on a low base produce an equally low calculation base for the transitional allowance.
- You may propose facilities; if they meet the objective social-medical criteria, the pension body assigns you one of them (§ 15(6a) sentences 1 to 3 SGB VI).
- The pension insurance provides medical rehabilitation in approved rehabilitation facilities (§ 15(2) sentence 1 SGB VI).
- The two-year route of § 11(2) does not apply to vocational participation benefits (Leistungen zur Teilhabe am Arbeitsleben) — it is reserved for prevention and medical rehabilitation; there § 11(1) and (2a) SGB VI apply.
- Which body is responsible depends on the case: for people in work without an occupational disease or a war-victim condition (Versorgungsleiden), the DRV states that it is the pension insurance.
Common mistakes
- Relying on the two-year rule while only voluntarily insured. It requires COMPULSORY contributions (§ 11(2) no. 1 SGB VI); voluntary contributions count only if they are treated as compulsory ones (§ 55(2) no. 1).
- Forgetting the medical documents. The medical report S0051 or the expert opinion is attached to the application — ask your body beforehand which one it needs.
- Leaving out annex G0110 to the application for medical rehabilitation, which the DRV form package lists alongside G0100.
- Applying for a second rehabilitation within four years without setting out the urgent health grounds (§ 12(2) SGB VI).
- Keeping quiet about an accident at work or an occupational disease. Where another body can provide equivalent benefits, the pension insurance is excluded (§ 12(1) no. 1 SGB VI).
- Taking the transitional allowance for granted. It requires that the benefit cannot be provided alongside the self-employed activity (§ 20(1) no. 1 SGB VI), and its calculation base is 80 per cent of the income contributions were paid on (§ 21(2)).
Frequently asked questions
Can a self-employed person get rehabilitation from the pension insurance at all?
Yes, if insured and the conditions are met: the personal ones in § 10(1) SGB VI and the insurance ones in § 11. Compulsorily insured self-employed people qualify most easily, through six calendar months of compulsory contributions in two years (§ 11(2) no. 1).
I only pay voluntary contributions — what applies to me?
The 15-year qualifying period of § 11(1) SGB VI, towards which voluntary contribution periods also count (§ 51(1), § 55(1)). Without it, medical rehabilitation remains open if your earning capacity is reduced or is expected to be in the foreseeable future and you have completed the general qualifying period (§ 11(2) no. 3).
Will I receive money during rehabilitation?
Transitional allowance, if the benefit cannot be provided alongside your work and you earned income from work immediately beforehand and paid contributions in the assessment period (§ 20(1) nos. 1 and 3 SGB VI). The base is 80 per cent of the income on which your contributions for the last calendar year were based (§ 21(2)).
How long does medical rehabilitation last?
It should be provided for three weeks at most, but may last longer where needed to reach the rehabilitation goal (§ 15(2) sentences 3 and 4 SGB VI).
Can I choose the clinic?
You may propose facilities (§ 15(6a) sentence 1 SGB VI). If they meet the objective social-medical criteria, the body assigns you one of them; otherwise it proposes facilities, among which you choose within 14 days.
What if rehabilitation cannot help?
If your earning capacity is reduced and success is not to be expected, or the benefit did not prevent the reduction, the rehabilitation application counts as a pension application (§ 116(2) SGB VI).
