The sworn statement of address (attestation de domicile) certifies where you live, when the requesting body accepts it instead of (or alongside) a utility bill or rent receipt. No CERFA form exists: generate your dated, signable statement below, for free (Word or PDF).

Deadlines

ID card or passport
The proof of address must be less than one year old on the day you file the application; if you are hosted, your host's statement certifies that you live with them on a stable basis or for more than 3 months (sheet F14807).

Documents to prepare

  • Valid identity document
  • Supporting document in the host's name if you are hosted by someone

What you get

  • The document as a PDF, ready to print and sign.
  • The same document in Word (.docx), so you can edit it before signing.

Free, no account and no email address.

The generated document is written in French — the language the receiving French authority expects. This page is in English; the document itself is not translated.

Generate your document

Free. The document is generated on demand: your entries are not recorded in any of our databases and are only used to produce it. They travel in the request body, never in the page address.

All fields are required unless marked “optional”.

You and your home

As written on your identity document.

Why we ask this

You are the one attesting to your address: your name opens the document and your signature commits you.

Number, street, postcode and city.

Why we ask this

This is the address the document certifies. It must match your other supporting documents, or the body will set the statement aside.

Living at this address since

The date you moved in. The document spells it out in French.

Why we ask this

How long you have lived there is often what the body wants established; the date goes into the attesting sentence.

For example: 27 3 1990

Place and date of signature
Why we ask this

The place of signature closes the document, the expected form of a French administrative writing.

Date

Usually today's date. The receiving body sets the accepted age: under one year for an identity card or passport.

Why we ask this

This is the date the receiving body looks at; it sets the accepted age (under one year for an identity card or passport).

For example: 27 3 1990

A draft is a file you save wherever you like, just like the document itself: we keep no copy of it and nothing is left in your browser. When signed in, you can also save it to your space to pick it up later — only if you ask.

Signature

Document drafted by ActioFin; it carries no legal standing before the tax authorities. Read it through and adapt it to your situation before signing.

This document is signed by hand, after printing: we do not offer electronic signature. Only a qualified electronic signature, issued by a certified provider, carries the same weight as a handwritten one (eIDAS regulation). A mere click would not — and letting you believe otherwise would expose you the day a body challenges it.

Who this applies to

  • Anyone who must evidence their address when a standard proof (energy bill, rent receipt, tax notice) is missing or not in their name.
  • Note: for an ID card or passport, the list of accepted proofs is regulated (sheet F14807) — check that a sworn statement is enough before relying on it alone.

Worked example

Example: Marie, a tenant with no bill in her name

Marie has just moved in with her partner and no utility bill is in her name. She writes a sworn statement of residence, dates and signs it, then attaches her partner's ID and a proof of address in his name. The prefecture accepts the file.

How to fill it in, step by step

  1. State your full name, date and place of birth.
  2. Give the complete address of the declared residence.
  3. Add the sworn wording confirming you live at this address.
  4. Date and sign by hand.
  5. Attach the supporting documents requested by the authority.

The template, item by item

Identity
Full name as on the identity document.

Date it, print it, sign by hand; attach ID if requested.

Good to know

  • A false statement is punishable by one year of imprisonment and a €15,000 fine (article 441-7 of the French Criminal Code).
  • For an ID card or passport, the proof of address must be less than one year old on the day you file the application (sheet F14807). For other procedures, the requesting body sets how recent it must be: check before sending your file.
  • The certificate is produced when an organisation asks for it: date it the day you write it.

Common mistakes

  • Confusing it with the hosting certificate: if you live at someone else's home, THEY attest (dedicated template linked below).
  • Using a sworn statement where a named document (bill, receipt) is required — check the accepted list first.
  • Declaring an address that is not your actual residence: a criminal offence (French criminal code art. 441-7).
  • Omitting the place and date — they determine whether the statement is admissible.

Frequently asked questions

Is a sworn statement enough as proof of address?

It depends on the procedure: some bodies accept it, others require a named document (sheet F14807 lists what is accepted for ID cards and passports).

I live at someone else's home — which template?

The hosting certificate, written and signed by your host — our dedicated template generates it.

How long is a sworn statement of residence valid?

It depends on the procedure. For an ID card or passport, the proof of address must be less than one year old on the day you file the application; if you are hosted, your host's statement certifies that you live with them on a stable basis or for more than 3 months. For other procedures there is no single period: the requesting body sets how recent it must be.

My electricity or gas contract is in my name — do I need to provide a bill for my ID card?

Not necessarily: with an EDF, Total Direct Énergie, Engie or Gaz Tarif Réglementé contract in your birth name, the Justif'adresse service automatically checks the address entered in the application. It is not available in the overseas collectivities (COM).

Can the administration refuse my proof of address?

Not if it is secured: a proof bearing a 2D-Doc barcode cannot be refused for an ID card or passport. A sworn statement carries no such barcode.
